Scheduling. Our scheduler was designed specifically for rehabilitation scheduling requirements. For patients requiring appointments across disciplines, it allows multiple appointments for one patient with multiple providers. It allows for comprehensive individual, group, location, and resource scheduling with recurring and staggered appointments -- including wait lists and bump lists. Referrals and Authorizations. Patients who need evaluations and treatment follow-ups are tracked. As patient are scheduled, appointments are linked to the appropriate referral by the scheduler. Upon check-in, the progress note/charge is created, with a link back to the referral. Upon signing and filing the progress note/charge, the therapist receives notification of the number of authorized visits and the expiration date. Incomplete referrals and those requiring additional authorization are fully tracked.

Patient Registration. When a patient arrives for the first time, the system gathers all pertinent demographic data. The patient record then stores all services the patient will receive.

Progress Note. An incomplete progress note is automatically generated at check-in, which reduces the potential for lost charges. The progress note/charge also provides information needed to generate productivity reports for the clinical staff. Patient progress is tracked and monitored from scheduling and check-in through evaluation, progress report and discharge.

Charge. The progress note includes a charge component with easy charge code look-up tables by code, description, category, or most-frequently-used. The progress note/charge manages Medicare time tracking and National Correct Coding Initiatives Edits (NCCI or commonly referred to as CCI Edits). Upon completing a progress note/charge ticket, the therapist is presented with a pop-up that displays number of scheduled appointments, number of remaining authorized visits, and an insurance authorization expiration date. From this pop-up, the therapist can request additional insurance authorization by changing the status of the referral and entering a comment. The therapist can also includes special notes to the scheduler as well as print a list of scheduled appointments.

Evaluations. There is a standard, discipline-specific evaluation forms library contains numerous sections with the ability to customize into sets and forms with free-form text. The pre-formatted material can also be an effective tool for educating new clinicians about your facility's standard for documentation and patient care. There are over 700 evaluation sections that can be included in diagnosis-specific evaluation sets with pre-populated goals. Designated data from the evaluation can be pulled into the Plan of Care.

Plan of Care. Plan of care reports, auto-generated from the evaluation, include recommendations, plans and goals for the patient as well as automatic assignment of Plan of Care due dates based on patient payer types. The system notifies the staff of incomplete or overdue plans of care. The status of a Plan of Care may be tracked, including attempts to obtain authorization from the physician. Plan of care reports may be electronically signed and faxed from the desktop.

Flow Sheets. Flow sheets provide electronic management of clinical data entry and review of patient progress over time. Flow sheets can accommodate multi-disciplinary documentation requirements and may be linked to progress notes/charges.

Reports and Analytics. Chart Links provides many standard reports to optimize the management of a rehabilitation facility. As the library of reports has been built based on real-life information needs of our customers, there are a multitude of meaningful reports available for use. A data dictionary may be provided to your information technology (IT) department so that ad hoc reports may be created. Alternatively, Chart Links provides an Export Wizard that allows you to determine the data to extract and compare. Chart Links also offers an add-on analytics / business intelligence application.

Outcomes. Chart Links is able to interface to most outcomes measurement providers. By measuring performance against providers all over the country, you can identify strengths and weaknesses to target areas for improving clinical practice and the continuing education of your staff. You can also gain access to information that will allow your facility to better market services, track outcomes of referral sources, negotiate contracts with payers, and better understand your patients. Compliance. Chart Links products are driven by major accreditation, profession, IT and security standards. We support compliance with JCAHO, CARF, AOTA, APTA, HIPAA and other rehabilitation professional standards. The system's documentation helps ensure compliance with Medicare CCI edits, modifiers and time tracking. Unlike paper forms unable to force standards compliance, Chart Links' electronic forms do not accept signatures without fulfillment of all required standards. Conformity with HL7, XML and the Security/HIPAA requirements is also uniform.

Interfacing. Chart Links is capable of interfacing to inbound Admission/Discharge/Transfer (ADT) for patient registration data and to Outbound Billing Systems. Interfaces also exist for Inbound Scheduling, Outbound Scheduling, Outbound Results, and Customized ADT or Billing.
